Estratégias baseadas em vesículas extracelulares de probióticos para o tratamento de psoríase – uma revisão integrativa

A psoríase é uma doença inflamatória crônica que acomete 2-3% da população global , caracterizando-se por lesões cutâneas persistentes, desregulação imunológica, e significativa redução na qualidade de vida dos indivíduos afetados. Apesar dos avanços terapêuticos , as opções disponíveis ,como tratamentos tópicos, sistêmicos e biológicos , enfrentam desafios relacionados à eficácia limitada, efeitos adversos e altos custos, destacando a necessidade de abordagens terapêuticas inovadoras. Nesse contexto, as vesículas extracelulares derivadas de probióticos (VEs ) têm emergido como uma estratégia promissora devido as suas propriedades imunomoduladoras e anti-inflamatórias. Os achados indicam que as VEs de cepas probióticos como Lactobacillus plantaram , L.casei , L.murinus e Propionibacterium freudenreichii apresentam capacidade de modular processos inflamatórios ao suprimir citocinas pró- inflamatórias (TNF-alfa , IL-17) e aumentar a produção de citocinas anti-inflamatórias (IL-10). Além disso , as VEs promovem a polarização do macrófagos para o fenótipo M2 , favorecendo a resolução da inflamação e a regeneração tecidual e modulam as vias de sinalização intracelular como NF-kB contribuindo para a restauração da homeostase tecidual.Esses mecanismos sugerem um relevante potencial terapêutico das VEs no manejo de doenças inflamatórias crônicas , incluindo a psoríase , e reforçam a viabilidade de seu uso em estratégias terapêuticas tópicas e personalizadas.
